If you cannot use the lowest ISO value or a flash, at least get exposure correct (or overexpose a bit) at the time of shooting. It will reduce the noise level in your image. Further noise ruduction can be done in Photoshop by applying noise filter selectively only to the areas that need it (via Layer Mask). This gives the best final result having sharp details and smooth out of focus backgrounds.
